Patient Information

Financial Policy

In an effort to control our billing costs, we have established the following policies regarding patient accounts:
We ask payment be made at the time of service, unless prior financial arrangements have been made (see payment plans).  For your convenience, we accept cash, checks, Visa, MasterCard, Discover Card, American Express and CareCredit.  Account balances that are over 60 days old will be subject to a 1.5% rebilling charge.  If you would like a written estimate of your treatment plan before starting treatment, please ask.

Dental Insurance

In most cases we will be able to file your insurance claims for you. There are many different types of dental insurance plans with a wide range of benefits.  Many of the plans will send payment of benefits directly to us.  Often we will be able to estimate how much of the charges your insurance company will pay.  We ask that you pay your estimated portion of the charges at the time of service, unless a payment plan has been established prior to treatment.  If your insurance company pays less than estimated, you will be billed for the difference.  If they pay more than estimated, you will credited or refunded the difference.  If you need to have a written explanation of benefits from your insurance company before treatment is started, please ask us to file a pre-treatment estimate for you.  There are some insurance companies that will not make payment of benefits to us (BC/BS, GHI, and Delta Dental, for example).  In those cases we ask that you pay the total charges at the time of service.  You will be reimbursed your benefit directly from your insurance company.  We will still be glad to file your claim.  Please remember that we are not a participating provider for any dental insurance plan.  Your insurance is a contract between you and the insurance company.  You have the final responsibility for payment for your treatment.

Payment Plans

For treatment plans over $300, we offer the following payment plans:

• If you pay the total charges the day treatment is started, you may receive a 5% pre-payment courtesy discount.  If you have insurance this only applies to the portion of the charges you are paying.

• You may pay in three equal installments.  The 1st is due the day the procedure is started.  The 2nd is due 30 days from the start date.  The 3rd is due 60 days from the start date.

• Longer payment plans are offered through CareCredit.  They offer 3 months,  6 months and 12 months 0% interest plans.  They also offer 24 months and 36 months plans that include interest charges.  If you wish to utilize a CareCredit plan, they must approve your credit application before treatment is started.

We would like to thank those who have always handled your account responsibly.  We assure you that establishing these policies are necessary so that we may continue to provide you affordable quality dental care.
